Document list (first cycle)

  1. Personal questionnaire (printed out and signed)
  2. Passport data
  3. A certificate of secondary education, with grades list attached
  4. Apostille or legalization of the certificate together with grades list
  5. Sworn translation of secondary education certificate into Polish language, or a translation certified by a Polish consul, together with grades list
  6. Information that the foreign certificate entitles the candidate to start post-secondary studies in the issuing country
  7. Sworn translation into Polish language, or a translation certified by a Polish consul, stating that the candidate’s certificate authorizes him/her to apply to University in the issuing country
  8. Authorization of the certificate by an Education Supervision Body. Does not apply to documents issued in EU, OECD, EFTA, EOG countries, or such countries with which Poland has agreements on education authorization
  9. Document confirming knowledge of English language; obligatory Skype interview
  10. Insurance policy against sickness and accidents for the period of candidate’s education in Poland, or European Health Insurance Card, or agreement with National Health Fund
  11. 3 up-to-date photos of the candidate (size: 35x45 mm; on the back of each of the photos there should be written the candidate’s name and additionally on one of them – permanent residence address

  3. A first cycle studies diploma, with grades attached
  4. Apostille or legalization of the diploma together with grades list
  5. Sworn translation of first cycle studies diploma into Polish language, or a translation certified by a Polish consul, together with grades list
  6. Information that the foreign diploma entitles the candidate to start post-secondary studies in the issuing country
  7. Sworn translation into Polish language, or a translation certified by a Polish consul, stating that the candidate’s diploma authorizes him/her to apply to University in the issuing country
  8. Recognition of diploma. Does not apply to documents issued in EU, OECD, EFTA, EOG countries, or such countries with which Poland has agreements on education authorization
  9. Document confirming knowledge of English language; obligatory Skype interview
